Data collected through the request form
- The form collects name, phone number, email (optional), contact preference, preferred clinic and service (optional) and, only if the patient fills it in, the reason for the request.
- The reason for your visit may contain health information: explicit, separate consent is required to send it.
- The website does not store submitted data: the request is forwarded to Dr. Greco by email.
- If the patient provides an email address, they receive an automatic message confirming that the request has been received, without any health information they may have described.
